Epidemiology staff collaborates with all Communicable Disease Division programs as well as other Department of Community Health Division, including; Environmental Health, the Public Health Laboratory, and the Nursing Outreach Division for communicable disease related:

- Education
- Prevention
- Control measures
The Epidemiology Staff also responds to inquiries from the public and other agencies regarding communicable disease issues and Epidemiology. |

Under Title 17, California Code of Regulation, Section 2500 (Division 1, Chapter 4, Article 1), all health care providers knowing of or in attendance on a case or suspected case of any communicable disease or condition must report to the local health officer for the jurisdiction where the patient resides. |

To report a communicable disease or condition, please fill out the Confidential Morbidity Report form and fax it to (559) 445-3535. |
